Wagner糖尿病足分級法，分為0到5級，

5級：壞疽影響到整個足部，病變廣泛而嚴重，部分發展迅速。
5疾病診斷
(一)病史採集
要了解糖尿病人患病持續時間、治療方式及其他並發症情況，識別腳出現潰瘍的原因、持續時間、程度和進展情況；
(二)體格檢查
時注意潰瘍面的外觀、范圍、深度、溫度、氣味，同時確定腳有無畸形、浮腫、軟組織感染或骨髓炎。檢查患者對側肢體情況及鞋襪是否合適；
(三)輔助檢查
1、神經系統檢查
目的是了解患者是否仍存在保護性的神經感覺。最為簡單和常用的方法是用一根特製的10克尼龍絲，一頭接觸於患者的大腳趾、腳跟和前腳底外側，用手按住尼龍絲的另一頭，並輕輕施壓，正好使尼龍絲彎曲，患者腳底或腳趾此時能感覺到腳底的尼龍絲，則為正常，否則為不正常。另外還有用音叉來檢查患者對振動的感覺。
2、皮膚溫度檢查
糖尿病足檢查皮膚對溫度變化的感覺，反應神經功能是否受損。分定性和定量檢查。定性即將音叉或一根細不銹鋼棍置於溫熱水杯中，取出後測定患者不同部位的皮膚感覺，同時與正常人對照。定量檢查需要用儀器。
3、壓力測定
通過測定腳不同部位的壓力，了解患者是否有腳部壓力異常。通常讓受試者站在有多點壓力敏感器的平板上，通過掃描成像，在計算機上分析
4、周圍血管檢查
最簡單的方法是用手來觸摸腳背或脛後動脈的搏動來了解腳部大血管病變，波動消失提示有嚴重的大血管病變，需進行下一步檢查。
(1)血管超聲檢查：通過檢查明確血管有無狹窄或閉塞。
(2)踝動脈-肱動脈血壓比值：反映下肢血壓與血管狀態，正常值為1。0-1。4；﹤0。9為輕度缺血，0。5~0。7為中度缺血，﹤0。5為嚴重缺血。嚴重缺血的患者容易導致下肢(或腳趾)壞疽。
(3)血管造影：了解下肢血管閉塞程度和部位，為截肢平面或血管旁路手術提供依據。
(4)跨皮氧分壓測定：反映微循環狀態，同時反映周圍動脈的供血狀況。
(5)血流變學檢查：全血粘度，血漿比粘度，全血還原粘度；紅細胞聚集指數，紅細胞剛性指數；血漿纖維蛋白原含量測定。
5、潰瘍合並感染的檢查
用探針探查懷疑有感染的潰瘍，如發現竇道，探及骨組織，要考慮骨髓炎；同時用探針取潰瘍深部的標本做細菌培養，增加培養出感染細菌的特異性。深部感染或骨病變還可用X線平片、同位素掃描或磁共振檢查等方法鑒別。
6、Charcot關節病的檢查。
長期糖尿病史患者可能並發Charcot關節病。需做專科檢查、確診。
6鑒別診斷
需與下肢脈管炎或血管炎、下肢神經病變等鑒別診斷。
真正的脈管炎：血栓閉塞性脈管炎，脈管炎是血栓閉塞性脈管炎的簡稱，是一種四肢中、小動脈慢性閉塞性疾病，其病理變化為中、小動脈血管壁的節段性、非化膿性炎症伴動脈血管腔內血栓形成，管腔閉塞引起肢體遠端缺血而產生疼痛。本病的主要特徵是：
（1）本病多發於男性青壯年；
（2）肢體特別是足趾發涼、怕冷、麻木和感覺異常是常見的早期症狀；
（3）疼痛是本病的主要症狀，表現為：
①間歇性跛行：當病人行走一段路程後，小腿或足部肌肉發生麻木、酸脹、疼痛、抽搐、無力等症狀，如果繼續行走則症狀加重，最後被迫止步，原地站立休息片刻後，疼痛迅速緩解，可繼續行走，但行走後上述症狀又復現。這種症狀稱為間歇性跛行，它是下肢動脈供血不足的典型表現。
②靜息痛：動脈缺血嚴重時，患肢疼痛劇烈而持續，休息時疼痛仍不止，徹夜難眠。甚至足趾破潰合並感染，疼痛更為劇烈。
老年人的「脈管炎」：下肢動脈硬化閉塞症，下肢動脈硬化閉塞症不是脈管炎，它是全身動脈硬化的一種表現，是中、老年人的常見血管病之一。其病理特點是腹主動脈、髂動脈、股動脈、腘動脈等大中動脈內膜增厚變硬，形成粥樣斑塊及鈣化，以及繼發血栓形成等，導致動脈管腔狹窄或閉塞，表現為與脈管炎類似的下肢缺血症狀，因此常被人們誤認為是脈管炎。也很多中、老年患者出現下肢疼痛、肌肉酸痛無力、不能正常行走（即間歇性跛行）等，常常以為是骨質增生、骨質疏鬆、腰椎間盤突出、風濕病等所至，服用了很多葯物久治不愈，未及時到醫院找專科醫生就診，甚至有些病人因此而延誤了就診時機而被迫截肢。
糖尿病足壞疽與其他壞疽的鑒別要點：壞疽是組織細胞的死亡。病因上常分為循環性壞疽，如動脈粥樣硬化性壞疽、栓塞性壞疽、血栓閉塞性脈管炎，雷諾病等引起的壞疽、神經營養性壞疽，糖尿病性壞疽，機械性，物理性，化學性，損傷及感染性壞疽等。糖尿病性足壞疽，單從病理變化及壞疽的性質、程度很難與其他壞疽相區別。尤其是中老年糖尿病患者伴發動脈粥樣硬化性壞疽時更難區分。但糖尿病足壞疽患者具有血管病變程度嚴重，病變進展較快，常伴有周圍神經病變及感染等特點。在臨床上還常可遇到足部壞疽久不癒合，檢查時才發現糖尿病的病例。應注意分析壞疽的發生，是伴發病還是合並症，加以區別。[1]

病情嚴重時可出現皮膚破潰、組織糜爛、直至壞死。還可因為皮膚麻木或感覺消失，對過熱、過冷物體感覺的不靈敏，而致燙傷或凍傷。
